Making an appointment
Patients must be referred to the Sleep Disorders Center by a physician in order to undergo a sleep study or treatment. Please print out the Referral Form and take it with you to your next physician's appointment. Completed forms can be faxed to 469-204-0273. If you are a physician referring a patient, please use the same form.
